The survey takes about 10-15 minutes and spans eleven sections: about you, what CIL services help you meet disability-related needs, what your community needs, barriers in your community, emergency preparedness and disaster response, meeting your daily needs, your thoughts, taking action with advocacy, getting connected with SILC, legislative priorities, and anything else.
